Information we collect

If you fill out a form on this site — for example, a callback request, a provider referral, or one of our sleep check-ins — we may collect your name, phone number, email address, and your answers to the questions in that form, which can include information about your sleep, your symptoms, or your child’s sleep and symptoms.
If you call our office, we collect the information you share with us on the call, along with your phone number.
We also collect some information automatically when you visit the site, such as your device type, browser, pages visited, and general location. See “Cookies & analytics” below.
How we use your information
We use your information to respond to your requests: to call you back, answer your questions, schedule appointments, and review referrals from other providers. Screener answers help us understand your situation before we talk.
We may also use your contact information to follow up about your inquiry and, with your consent, to send occasional updates and offers from the practice by text or email.
We do not sell your personal information.
Forms, screeners & phone calls
The sleep check-ins on this site are self-checks, not medical diagnoses. Your answers are used to help our team understand your concerns and prepare for a conversation with you. Submitting a form or completing a screener does not make you a patient and does not create a doctor–patient relationship.
Information you provide through this website is not the same as your medical record. Once you become a patient, your health information is protected under our separate patient privacy practices and, where applicable, HIPAA. Referring providers may send us patient records by fax (385.284.1906) or email ([email protected]) — please don’t submit patient records through this website’s forms.
Cookies & analytics
This site is built on a website platform (GoHighLevel) that may use cookies and similar technologies to make the site work and to understand how visitors use it — for example, which pages are visited most.
We do not use third-party advertising trackers on this site.
You can control cookies through your browser settings. Blocking cookies may affect how parts of the site work.
Sharing & third parties
We share information only as needed to run the practice:
Service providers. Companies that host our website and forms (GoHighLevel), handle our phone system, or help us operate — they may process your information on our behalf and are not permitted to use it for their own purposes.
Referring providers. If another healthcare provider referred you to us, or you asked us to coordinate with your provider, we may communicate with them about your care.
Legal requirements. We may disclose information if required by law, such as in response to a valid legal request.
We do not sell or rent your personal information to anyone.
Your rights & choices
You can ask us what personal information we have about you, ask us to correct it, or ask us to delete it, subject to legal and record-keeping requirements. To make a request, call 801.960.9667.
If you receive marketing messages from us, you can opt out at any time — reply STOP to any text, use the unsubscribe link in any email, or call us and we’ll remove you.
Data retention & security
We keep the information you submit for as long as we need it to respond to you, provide care, and meet our legal and professional obligations, then delete or de-identify it.
We use reasonable safeguards to protect your information, and our service providers do the same. That said, no website or transmission method is 100% secure, so we can’t guarantee absolute security — please don’t submit anything through the site that you’d only want in your confidential medical record.
Children’s privacy
Our website is intended for adults. Parents and guardians may submit information about their children — for example, through the child sleep check-in — and when they do, we treat that information with the same care described in this policy. We do not knowingly collect information directly from children under 13 online. If you believe a child has submitted information to us directly, contact us and we will delete it.
Changes to this policy
We may update this policy from time to time. When we do, we’ll change the “Last updated” date at the top of this page. Significant changes may also be noted on the site. Your continued use of the site after changes means you accept the updated policy.
